How We Assist You
Our Community Nursing supports are delivered by qualified registered nurses and are tailored to your individual clinical needs, health goals, and NDIS plan.
Medication Administration & Management
Safe administration and management of prescribed medications including complex medication regimes with accurate documentation and GP coordination.
Wound Care & Dressing Changes
Assessment, treatment, and monitoring of acute and chronic wounds, pressure injuries, and post-surgical sites, following current evidence-based clinical guidelines.
Chronic Disease Management
Ongoing monitoring and nursing support for participants living with chronic conditions such as diabetes, COPD, heart disease, epilepsy, and neurological conditions.
Continence Assessment & Support
Comprehensive continence assessments and the development of individualised continence management plans, delivered with sensitivity and respect for your dignity.
Health Monitoring & Clinical Assessment
Regular health checks, vital sign monitoring, clinical observations, and early identification of changes in health status keeping you and your care team informed.
Care Coordination & Liaison
Liaison with your GP, specialists, allied health professionals, and NDIS support team to ensure your clinical care is coordinated, documented, and aligned with your goals.

Who is this service for?
This service is for NDIS participants who have a disability-related health support need that requires the skills of a registered nurse. This includes participants with complex medication needs, chronic health conditions, wounds requiring clinical management, continence needs, or those requiring regular health monitoring. Participants of all ages are welcome including children and young people with complex health needs. We accept NDIA-managed, plan-managed, and self-managed participants.
Are your nurses qualified and registered?
How do you coordinate with my GP and other health professionals?
Coordination with your broader healthcare team is central to how we work. With your consent, we communicate directly with your GP, specialists, and allied health professionals sharing clinical updates, flagging concerns early, and ensuring everyone involved in your care is working from the same information. We treat your GP as a key partner, not an afterthought.
